Root Cause

A root cause is the underlying reason or source of a problem or issue. It is the fundamental factor or factors that contribute to the occurrence of a particular problem or issue. Identifying the root cause of a problem is important for developing effective solutions and preventing the problem from recurring in the future. Root cause analysis is a method used to identify the underlying causes of problems, often involving a systematic investigation of the problem and the development of solutions to address the identified root causes. In many cases, the root cause of a problem may be hidden or difficult to identify, and may involve multiple factors or processes that interact in complex ways. In order to address a problem effectively, it is important to identify and address the root cause, rather than just treating the symptoms of the problem. This can involve a collaborative effort between stakeholders, including subject matter experts, management, and affected parties, in order to gain a comprehensive understanding of the problem and its underlying causes.
